NS Oral and Dental Health Clinic Services Ltd. Sti. requests your explicit consent regarding the processing and transfer of your data as detailed in the Information Notice on the Processing of Personal Data, except in cases where processing is necessary for the performance of the contract, compliance with legal obligations, protection of public health, preventive medicine, medical diagnosis, treatment and care services, or the planning and management of healthcare services and financing. 

The personal data obtained primarily for conducting all medical diagnosis, examination, treatment, and care services, including but not limited to the following, may be processed and transferred as needed: 

Identity Data: Your name, surname, ID number, passport number, place and date of birth, marital status, gender, and other identity information obtained through the ID card or similar identity documents you provide. 

Contact Data: Address, phone number, email address, and other contact information. 

Financial Data: Bank account number, IBAN, and other financial data. 

Health Data: Laboratory and imaging results, test results, examination data, prescription information, and other health and sexual life data obtained during the provision of medical diagnosis, treatment, and care services, as well as the responses and comments you share to evaluate our services. 

Digital Data: Camera recordings taken during your visits to our hospitals, and audio recordings of your calls with our call center. 

Other: Data related to your private health insurance and Social Security Institution for financing and planning health services, browsing information, IP address, browser details, and other data such as medical documents, surveys, forms, and location data that you voluntarily share with us during your use of our website and mobile application. 

We inform you that your personal data and special category personal data mentioned above may be processed for the following purposes: 

• Protection of public health, preventive medicine, medical diagnosis, treatment, and care services, 

• Sharing of requested information with the Ministry of Health and other public institutions and organizations as per applicable legislation, 

• Fulfillment of legal and regulatory requirements, 

• Financing your health services, covering diagnostic, treatment, and care expenses, and sharing requested information with private insurance companies under the entitlement inquiry, 

• Informing you about your appointment through our Call Center and Digital Channels, 

• Verification of your identity by the Patient Services, Healthcare Personnel, and Call Center departments, 

• Planning and management of internal operations by the Hospital Management, 

• Analysis for improving health services by the Quality, Patient Experience, and Information Systems departments, 

• Monitoring and prevention of fraud and unauthorized transactions by the Audit and Information Systems departments, 

• Execution of risk management and quality improvement activities by the Quality, Patient Experience, and Information Systems departments, 

• Invoicing for your services by the Patient Services, Financial Affairs, and Marketing departments, 

• Confirmation of your relationship with institutions partnered with your hospital by the Patient Services, Financial Affairs, and Marketing departments, 

• Responding to any inquiries and complaints regarding the healthcare services provided or to be provided by the Hospital Management, Patient Experience, Patient Rights, and Call Center departments, 

• Taking all necessary technical and administrative measures to ensure the data security of your hospital systems and applications by the Hospital Management and Information Systems departments, 

• Participation in campaigns and the provision of campaign information, designing and delivering special content, tangible and intangible benefits on web and mobile channels by the Marketing, Media, and Communication, Call Center departments, 

• The Hospital Management, Patient Rights, and Patient Experience departments measured, enhanced, and researched patient satisfaction. 

We inform you that the personal data and special category personal data mentioned above may be stored in physical and electronic archives within Dente Dental Studio and external service providers with utmost care and compliance with legislative provisions. 

Regarding the transfer of personal data, your personal data may be shared, by the provisions of the Health Services Basic Law No. 3359, Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliates, the Personal Data Protection Law No. 6698, the Regulation on Private Hospitals, the Regulation on the Processing of Personal Health Data and the Protection of Privacy, and other relevant legislation, with the following institutions and for the purposes mentioned above: 

• The Ministry of Health, its sub-units, and family medicine centers, 

• Social Security Institution, 

• General Directorate of Security and other law enforcement agencies, 

• Directorate General of Population and Citizenship Affairs, 

• Turkish Pharmacists’ Association, 

• Judicial authorities, 

• Your legal representatives with your authorization, 

• Private insurance companies, 

• Third parties, including lawyers, tax advisors, and auditors with whom you work, 

• Another hospital to which the patient is referred or self-applied, 

• Regulatory and supervisory institutions and official authorities, 

• Suppliers, support service providers, archive service providers, and business partners with whom we cooperate or from whom we benefit as a company. 

Method and Legal Basis of Collecting Personal Data: 

We inform you that your personal data is collected and processed by Dente Dental Studio through all kinds of verbal, written, visual, or electronic means for the above-mentioned purposes and in line with the activities of Dente Dental Studio, within the framework of legal boundaries, to fully and properly fulfill the contractual and legal obligations of Dente Dental Studio. 

I have read the above information and been informed in detail by Dente Dental Studio about the legal basis for collecting my personal data, as explained above: 

• Personal Data Protection Law No. 6698, 

• Health Services Basic Law No. 3359, 

• Decree-Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliates, 

• Regulation on Private Hospitals, 

• Regulation on the Processing of Personal Health Data and the Protection of Privacy, 

• Ministry of Health regulations and other legislative provisions. 

Furthermore, as stated in Article 6, paragraph 3 of the Law, I am aware that personal data relating to health and sexual life may be processed without my explicit consent only to protect public health, preventive medicine, medical diagnosis, treatment, and care services, and the planning and management of healthcare services and financing, by persons or authorized institutions and organizations who are under a confidentiality obligation. 

Rights Regarding the Protection of Personal Data: 

I have been informed that I have the right to: 

• Learn whether my personal data has been processed, 

• Request information regarding the processing of my personal data has been processed, 

• Access my personal health data and request a copy, 

• Learn the purpose of processing my personal data and whether it has been used by its purpose, 

• Know the third parties to whom my personal data has been transferred, either domestically or abroad, 

• Request correction of my personal data if it has been incompletely or inaccurately processed, 

• Request deletion or destruction of my personal data, 

• Request notification of third parties to whom my personal data has been transferred regarding the correction or deletion of my personal data, 

• Object to the occurrence of any adverse result from the analysis of my processed data exclusively through automated systems, 

• Request compensation for damages in case of unlawful processing of my personal data.
